If you have, say the deck discard LS and you put it in a site and make an opponent discard the top card of his deck and than your opponent uses the Wanderer LS and puts it in his Site would you have to discard the top card of your deck also?

If the Wanderer soul was not already in a site, and your opponent swapped for your Deck Discard and then placed DD in a site, yes you would have to discard.
If the Wanderer is in a site already, exchanging does not count as "putting the soul in a site."

False, putting a LS in a site means putting a LS in a Site. You may use Exchanger already in a site to activate a "when put in a Site" LS ability.

I've always had it ruled (including at Nats 2011, though I don't remember the specific judge) that "putting a Lost Soul in a site" can only be achieved by 1) putting it in an unoccupied site during your Prep Phase per game rule, or 2) using a special ability that lets you put a soul in a site. I've specifically been told that exchanging with a Wanderer already in a site does not trigger these Souls.
Also, exchanging one of the Judges angels into discard pile is NOT considered a discard of that angel, so why is this exchange different?

The REG says it's only a discard if the card ended up on the top of the draw pile.
Site Maintenance
Site Maintenance is the time when Lost Souls are placed in sites. This happens during your PREPARATION PHASE. (Please read Sites under Preparation Phase in the rulebook for additional information).
So yep, exchanger works, because it's still prepped.

Except that it's been ruled the exchange isn't a discard even if you're exchanging for the top card.
It seems inconsistent to rule one thing as equal to another and not something else when it's the same ability (though the whole add to battle sometimes equals band thing doesn't inspire confidence in consistency winning out here).
