If leprosy is placed on a hero and the hero then makes a rescue attempt and is killed in battle does leprosy get discarded along with the hero or does it move on to another hero in the territory?

Leprosy, sa: "Selected Hero decreases 0/2 per turn. If Hero reaches */0 or less, discard Hero. Then Leprosy is placed on another hero in that territory and continues,"

It says "If Hero reaches */0 or less, discard Hero," so I would think that the "then" clause that follows only applies when the hero's defense is reduced to 0 or less.

Correct. If a leper dies, his disease doesn't carry onto people he never touched.
