If I block with Spirit of Temptation (http://redemption.wikia.com/wiki/Spirit_of_Temptation_(TP)) and instead of placing an enhancement underdeck place it on Widows' Table (http://redemption.wikia.com/wiki/Widows%E2%80%99_Tables_(TEC)), does my opponent still have to underdeck a card?

By definition of "Instead," you did not actually underdeck a card, therefore neither would your opponent, IMO.

Although this actually leads me to wonder if they must place an enhancement on Widow's table instead as well, since it says opponent must do the same, and not that opponent must underdeck a card..

I ask because it doesn't seem to be a cost/benefit ability based on its wording.

"The same" refers to the use of the SA in the first sentence. Since the underdeck never happened, there is no "same" action to be matched. Widow's Table cannot be activated by the opponent.

YMT is correct. It isn't worded as cost-benefit with a "to" but to do the "same" that first thing must have occurred (as written).
