I have Plagued with Diseases active and my opponent has the most LSs and Josiah's Covenant active. He attacks with spy. Does he get to look at my hand and retreat back to territory before the game rule discard due to decrease kills him? What if he had attacked with Rhoda instead?
Plagued with Diseases - Decrease all opponents’ Heroes by 0/2 (or 0/3 if you have the fewest Redeemed Souls).
Josiah's Covenant - Protect all Heroes in player's territory from harm or negative effect.
Spy - 3/2 - Holder may look at one opponent's hand or cards face down in a site. Hero may then withdraw from battle unharmed or continue the battle.
Rhoda - 2/2 - Look at the top card of your draw pile. You may withdraw Rhoda from battle.

I would think in either case they'd get to withdraw without being discarded.

Correct. Plagued with Diseases cannot insert itself between Spy's two abilities. If Spy withdraws, he is back in territory where is protected.
