Question 1 -
The cards in question: Artifacts that do not specify "discard this to", but have limited uses.
Card examples: Covenant of Noah, Unsuccessful
The question: Are they discarded by game rule as soon as you use them, or at the end of the phase, or some other time?
Practical example: I play a battle winner on my evil character, my opponent says they're going to use Cov of Noah to negate my enhancement, can I chain Lying Unto God to negate his Covenant before it negates my battle winner?

Answered.

Question 2 -
Cards in question: Set-aside enhancements, heal enhancements, Territory-class enhancements, Dominants, etc.
Card examples: Pentecost, Torment, Angel of the Lord, etc.
The question: Do the above-mentioned cards actually physically enter play when you use them?
Practical example: Can my opponent use Cov of Noah to negate my evil set-aside, heal, or territory-class enhancement that I play during my preparation or discard phase?

Question 3 -
Is the Errata on Cov of Noah (see below) correct? If so, why does Unsuccessful not have a similar Errata?

Referenced Card Abilities -
Covenant of Noah (Pa)
Type: Covenant • Brigade: Blue • Ability: 2 / 5 • Class: None • Special Ability: Use as an enhancement or an Artifact. Interrupt and prevent one evil enhancement. Discard after use. • Errata: Use as an enhancement or an Artifact. You may discard this card to negate one evil enhancement.

Lying Unto God (G)
Type: Evil Enh. • Brigade: Multicolor • Ability: 2 / 3 • Class: none • Special Ability: Negate and discard an active artifact.

Unsuccessful (Pi)
Type: Curse • Brigade: Gray • Ability: 0 / 6 • Class: None • Special Ability: Negate the last good Enhancement played in battle (may be used once). • Play As: When holder chooses, negate the last good Enhancement played in battle (may be used once).

1.) As soon as it's used.
2.) I think so.
3.) I'll leave this one to the Elders.

1- I believe as soon as you use them. I didn't find this rule anywhere, and you'll probably want to confirm, but that is how multiple elders I've played against treated Holy Grail/Captured Ark, so I assume it is correct.
2- They definitely enter play. However, I believe they leave play before your Covenant could have a chance to negate them.

#2 Isn't that the whole point of an interrupt?

The point of an interrupt is to interrupt any card in play (or some other location if specified). I'm fairly certain that when you play a territory it activates and then says "hey, time to get discarded unless I'm told to do something else" and is discarded before you can use your covenant. By the time you can use your covenant, it isn't in play so it can't be targeted. Similarly, DON can't negate Unholy Writ after it has been used.

Atleast, this is what I think. I have no basis for this.

#1 - Exhausted artifacts are discarded immediately and cannot be negated.  source

Sad, okay. Thanks!

Since I don't want to try to answer the first two and be wrong, I'll take a shot at #3. Lying Unto God doesn't say to discard it anywhere in the ability, unlike Covenant of Noah. You can play it, negate and discard an active artifact, and it remains in battle. My guess is that Covenant of Noah has the discard portion because it can be used as an artifact, and that's the way to make sure it's not used repeatedly.

Hopefully someone else can help you with #1 & 2! ;D
I think you got Lying Unto God and Unsuccessful confused, but thanks for trying. :P
