Prisoner Transfer lets you move a captured hero in your territory to someone else's territory giving you one less Soul to protect and them one more Soul for you to have access to.

He asked if it meant replace, and so I explained that it doesn't; you're either moving or trading.

I think this is a case where you can just use the English definition to figure out what to do. To transfer means to "convey or remove from one place, person, etc., to another". I think it's fairly straightforward.
