What does "Select booster pack prizes, etc, etc" mean?
Do I pick up to five different packs, or one to receive five packs?

You can pick any combination you want as long as the total adds up to the total prize packs you will be giving away for your tournament.

ETc. a 2 category local will get 10 packs (5 for each category).  You can get these 10 in any combination you want. ( I reccomend the 10 unlimited packs  ;) )
